Basic Service Model
The Basic Service Model
provides a comprehensive assessment, short-term counseling of up to 5 sessions, and referral if necessary. This
option does not give the flexibility of treatment that 10 sessions provides. However, much work can be done
in 5 sessions, and customers often find that this model suits their needs. A range of presenting problems, generally
60 percent can be helped within this model without the need for a referral into their insurance benefit.
Professional services included within the Basic Model:
- Confidential assessment, brief counseling of up to 5 sessions, with case management
services
- Telphone counseling
- Assigned CONCERN professional staff member who serves as the primary liaison for communication with
designated company representative to review utilization, assess progress , identify changing needs, look at program enhancement , develop strategies, resolve problems,and
review recommendations for program improvement
- Letter of program introduction and brochures for household mailing
- EAP orientation for all employees, which increases trust and utilization
- Promotional material provided on a monthly basis
- Supervisory training for job performance and fitness-for-duty issues, which assists supervisors in
using CONCERN as a tool to identify and manage employees with performance problems
- Unlimited attendance at our quarterly supervisory training and 2-hour Employee Enrichment Series
(i.e. Managing Workplace Conflict, Stress Management, Balancing Work and Home, etc.) offered at a TriHealth
location
- Consultation with supervisors regarding employees experiencing problems that can or are impacting
their job performance
- Consultation with management in planning a significant organizational change that will have subsequent
impact on employees and their family members
- 24-hour telephone crisis-line
- Emergency counseling sessions are available Monday-Friday on an as needed basis
- Critical/Post-Trauma Stress Debriefing at the company site
- Substance Abuse Professional (SAP) assessment for regulatory compliance
- Semi-annual Utilization
