CONCERN^® Behavioral Health Management

CONCERN^® credentials independently licensed practitioners to participate on the managed care panel for contracted employer customers. For current panel providers, this site contains all the forms and information to assist you with meeting patient record requirements. In addition, we have included the Provider Manual, UM Criteria, treatment guidelines and educational materials for your patients that you may copy and distribute. We continue to strive to support your work with patients and to promote collaborative relationships with you. Please let us know how you think we are doing by filling out our provider survey.

For potential providers, please see the screening application in the Forms section. We constantly review the panel for specific needs with respect to expertise or location.

CONCERN Behavioral Health Management manages the behavioral health benefits for the following Cincinnati area employers: Procter & Gamble, General Electric, Cincinnati Public Schools and TriHealth. All care performed by a behavioral health provider under the insurance benefit must be preauthorized by CONCERN. To obtain authorization, please have the client call CONCERN at 513-891-1691 or 1-800-642-9794.
